CFSS in home services
Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS will replace personal care assistance (PCA) and the Consumer Support Grant (CSG). Only accepting referrals where client is coming with staff.

CFSS Consultation Services
Consultation services providers:
Provide education to help people make informed decisions about how to meet their needs using CFSS.
Help people write their service delivery plans, if desired.
Review people’s service delivery plans.
Provide people with ongoing support, as needed.

24 hours Emergency Services
On-call counseling, on-call problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency that is not related to an injury or an immediate danger. This service does not replace the use of 911 when necessary. Only accepting referrals for tier 3 or higher.

IHS with Training
Skill-building and instructional services to acquire, retain and improve the person’s experience living in the community. Training must be within an allowable community living service category and meet identified needs specified in the person’s assessment. IHS with training is a service option that provides support and training. The person must receive training in at least one of the community living service categories. They also may receive support in any of the community living service categories. This service option is available to adults only. A minimum of 5 hours week is required.

IHS without Training
IHS without training is a service option that provides support. The person must receive support in at least one of the communitity living service categories. This service option is available to adults and children. A minimum of 5 hours week is required.

IHS with Family Training
IHS with family training is a service option that provides support and training to the person and/or their family. The person and/or their family must receive training in at least one of the community living service categories. The training increases their capabilities to care for and maintain the person’s ability to live in the home. The person and/or their family also may receive support in any of the community living service categories. This service option is available to children and adults living in their family’s home.
This service option is available to adults when the adult and their family needs support and training in the family’s home in at least one of the community living service categories.
This service option is available to children when either of the following is true:
Child needs support and training in at least one of the community living service categories.
Child and their family needs support and training in at least one of the community living service categories.
A minimum of 5 hours week is required.

Homemaking Services
Offering support with household tasks, including cleaning and home management, to maintain a safe and comfortable living environment. A minimum of 10 hours a week is required.

Respite
Temporary care for individuals with disabilities, allowing their caregivers to take breaks. Accepting Client that come with their own respite staff.

Individual Community Living Supports (ICLS)
Providing assistance, reminders, cues, and supervision to help individuals remain in their own homes. A minimum of 10 hours week is required.

Ensuring overnight safety and support through awake staff presence.
